Reports

Regular medication reviews are an important part of comprehensive psychiatric care.

Where appropriate, medications may be carefully reduced or discontinued if they are no longer providing benefit, are causing side effects, or if treatment goals have changed.

Any medication changes are made collaboratively, using a gradual and evidence-based approach, with ongoing monitoring to support safety and wellbeing.

Medication Review and Deprescribing

The initial assessment is conducted over three appointments (45min-1h) allowing time to develop a thorough understanding of each individual's needs and circumstances.

This comprehensive assessment includes:

  • Exploration of current concerns and symptoms

  • Developmental, family, social, and medical history

  • Review of physical health and overall wellbeing

  • Collaborative psychiatric formulation, including collateral information where appropriate and with consent

Following assessment, an individualised treatment plan is developed in partnership with the client. Recommendations may include:

  • Medication management

  • Psychological therapies

  • Psychosocial interventions and supports

Regular follow up appointments ensure care remains evidence-based, responsive, and aligned with each client's evolving needs and treatment goals.
